Who We Are
Home Nursing Foundation (HNF) provides integrated and comprehensive care services to patients in the comfort and familiarity of their own homes. These services consist of home-nursing care, home-medical care and social welfare. For home-therapy and social support services such as food delivery and personal hygiene care, HNF links patients to appropriate community resources through our trusted collaborations.


Overview of Services
The Home Nursing Foundation (HNF) is the oldest and largest home-nursing service provider in Singapore. We provide integrated home-care services to patients in the comfort and familiarity of their homes. These services consist of home-nursing care, home-medical care and social welfare.

For home-therapy and social support services such as food delivery and personal hygiene care, HNF links patients to appropriate community resources through our trusted collaborations.

When patients seek our assistance or are referred to us, our nurses carry out an assessment of the patient’s nursing care needs. Our nurses will then work with the patient and his family to develop a care plan that best matches his requirements with his home environment. Our experienced nurses also conduct caregiver training for the main caregiver on how to care for the patient between each home visit.

HNF believes that with proper care and care management, patients are able to recuperate and remain independent or continue to be cared for in the comfort and privacy of their homes. Each patient’s needs are our primary concern. Our services are delivered to patients in a way which preserves their dignity, with integrity and respect for cultural, linguistic and social differences.


Volunteer
For the Financial Year 2012/13, HNF had an average of just under 2,000 active patients. Some of our patients either live alone or have limited family support.

Be part of our vibrant community of volunteers and make a difference to our patients’ lives. You can be a friend to our patients, bring them on excursions, buy and deliver goodie bags to patients’ homes or fulfil their simple wishes for basic necessities.

Volunteer with HNF :-

Organising Outdoor Activities
Organising outings and excursions for both wheelchair-bound and mobile patients. Outings can involve simple fun activities or celebrating festive seasons with our patients.

Getting Involved in Various Outreach & Fundraising Projects
Organising and executing fundraising projects and initiatives for HNF. You can propose your ideas to us if you are interested to hold your own fundraising event in support of HNF and our patients. You can also help out in the outreach and fundraising projects organised by HNF.

Providing Administrative and Logistics Support
The activities can include packing and distributing goodie bags, helping out at road shows and events, providing transport to collect sponsored items, and more.

Fulfilling Patients’ Wish List
Fulfilling patients’ wishes for necessities such as household products and medical supplies, which they may not be able to afford and will need help in buying, given their limited mobility.

Sponsoring Goodie Bags
Sponsoring goodie bags of daily necessities such as food, toiletries, and household cleaning and disinfectant products.

Adopting HNF as a Charity

Donate
Not all our patients can afford long-term nursing care. For the Financial Year 2012/13, about 65% of our patients are from the lowest income groups, with many relying on financial assistance. Some of them either live alone or have limited family support.

HNF believes that no patient should be turned away because of his/her inability to pay. However, none of what we do or wish to do would be possible without the financial help of the government and our generous donors. With donations and government subsidies, our patients can receive free or subsidised:

  •     medical and nursing help;
  •     medical consumables;
  •     consumables e.g. milk feed, diapers; and
  •     necessary items which they cannot afford e.g. fan, mattress, bar refrigerator.

We need your help to continue helping our needy and elderly housebound patients receive quality care in the comfort of their own homes.


Make a Donation and Support HNF
You can help HNF to serve our patients by making a vital donation. Your gift will go towards various areas of need such as subsidised nursing services, medical supplies and medicine for our patients.
